The effects of vault drainage through foley’s catheter on post-operative morbidity after non-descent vaginal hysterectomy

Abstract:
Introduction: Hysterectomy is a frequently performed gynecological procedure used to address benign conditions such as abnormal uterine bleeding, uterine prolapse, or fibroids. Vaginal hysterectomy (VH) is the preferred method over abdominal hysterectomy due to lower risk. However, VH still presents a significant risk of vault hematomas, and surgical drains are employed to alleviate fluid build-up at the surgical site. The study seeks to evaluate the efficacy of utilizing a Foley catheter in patients undergoing non-descent vaginal hysterectomy (NDVH).
Materials and Methods: It was a prospective study conducted over 1 year comparing the use of a drain (Foley's catheter) in with and without drain patients undergoing NDVH. All women undergoing NDVH for adenomyosis, fibroid uterus, or other benign conditions were included and women who had hysterectomy for malignant conditions, as well as those with rectocele or cystocele conditions, were excluded from the study.
Results: Patients who had a drain inserted experienced a reduction in post-operative complications such as fever, serous vaginal discharge, hematoma formation, and abdominal pain. Additionally, the use of a drain led to a decrease in hospital stay duration and the need for hospital readmissions when compared to patients without a drain.
Conclusion: Utilizing a vault drain following NDVH reduces febrile illnesses and hematoma formation, thereby decreasing postoperative febrile morbidity. Patients with a drain experience shortened hospital stays and reduced readmissions, along with decreased antibiotic usage
