Outcome of Induction of Labour by Intravaginal Misoprostol in Mymensingh Medical College Hospital, Mymenshingh, Bangladesh

Abstract: Induction of labour is a standard obstetric approach in properly selected patients. It is a method by which initiation of uterine contraction prior to their onset leading to cervical dilatation and effacement and delivery of a baby at third trimester and last two weeks of second trimester at that time baby become viable. To evaluate the safety and efficacy of vaginal misoprostol for induction of labour. A case control, randomized trial was carried out in 126 pregnant women admitted in Mymensingh Medical College Hospital, during the period of April 2009 to September 2009. Women who had unfavorable cervix (Bishop's score <6) were selected. 63 of them received 25μgm misoprostol intravaginaly for four doses 4 hourly. Another 63 patient received intravenous oxytocin starting from 10mU/ml up to 40 mU. 29 patients out of 63 patients of misoprostol group achieved normal vaginal delivery. Rest 34 patients ended by caesarian section. Time interval from induction to labour pain in misoprostol group was average 2 hour 14 minutes. The maximum required dose was 100μgm. Maternal complications were Nausea 4.8%, Hypertonicity - 6.3% and others - 4.8%. Neonatal outcome were satisfactory. 18 patients out of 63 patients of oxytocin group achieved normal vaginal delivery. Rest 44 patients ended by caesarian section. Neonatal outcome were satisfactory. In conclusion we can say Intra vaginal misoprostol is well tolerated and effective than intravenous oxytocin for induction of labour in unfavorable cervix. It certainly reduces the number of caesarean section. It is cost effective.
