Management of erysipelas in pregnancy: A case series of 10 patients in a secondary maternity type 2B setting

Abstract: Background: Erysipelas, a bacterial skin infection typically caused by Streptococcus pyogenes, is rare in pregnancy but can lead to significant maternal and fetal complications if mismanaged. Effective management in secondary maternity type 2B hospitals, which balance high-risk obstetric care with resource constraints, is critical. Objective: To describe the clinical presentation, management, and outcomes of 10 pregnant women with erysipelas in a secondary maternity type 2B facility. Methods: A retrospective case series of 10 pregnant women diagnosed with erysipelas between January 2022 and December 2024 at a secondary maternity type 2B hospital. Data on demographics, clinical features, treatment, and maternal-fetal outcomes were extracted from medical records. Results: The cohort had a mean age of 28.4 years (range: 22-34). Erysipelas primarily affected the lower limbs (80%). All patients received antibiotics (penicillin or erythromycin), with 70% requiring hospitalization (median stay: 4 days). Complications included cellulitis (20%) and preterm labor (10%). Cesarean delivery was performed in 30% of cases. No maternal or neonatal deaths occurred, though one preterm neonate required tertiary care transfer. Conclusion: Erysipelas in pregnancy can be effectively managed in a type 2B maternity setting with prompt antibiotic therapy and multidisciplinary care, yielding favorable maternal-fetal outcomes. Vigilance for preterm labor and neonatal risks is essential.
