Septate uterus with double cervix: A case report of a rare müllerian anomaly

Abstract: Septate uterus is the most common congenital Müllerian duct anomaly, resulting from incomplete resorption of the uterovaginal septum and accounting for nearly half of uterine malformations. The presence of a septate uterus associated with cervical duplication represents a rare anatomical variant, posing diagnostic and therapeutic challenges. We report the case of a 28-year-old nulligravida woman who presented with chronic pelvic pain, severe dysmenorrhea, and irregular menstrual cycles. Clinical examination and imaging with transvaginal ultrasonography and pelvic MRI revealed a complete septate uterus with two cervices and a normal external uterine contour, represents a rare developmental variant that does not fit neatly into existing ASRM MAC2021 diagnostic criteria. The patient underwent hysteroscopic septal resection with diagnostic laparoscopy, which confirmed normal fundal contour and bilateral tubal patency. Septal resection was successfully performed while preserving the cervical septum to avoid stenosis. Postoperative recovery was uneventful, with significant improvement in symptoms and normalization of menstrual flow at six-week follow-up. This case emphasizes the importance of recognizing rare Müllerian anomaly variants and highlights the role of advanced imaging and minimally invasive hysteroscopic management in restoring uterine anatomy and improving reproductive potential.
